MHH Massage Forms
If you've booked with MHH Massage for the first time, you should have received a booking confirmation with details about your upcoming session. The booking confirmation includes a request for "New Clients" to complete our "MHH Massage New Client Intake". This form needs to be completed prior to your arrival for therapeutic massage. Some individual's may need to complete specific forms that correspond with the type of session you booked with us. Follow the instructions below to complete your appropriate massage intake form.
Starr Fit Club Forms
If you've attended a complimentary consultation known as our "Wellness Discovery Meet" (zoom) with coach Starr of Starr Fit Club to inquire on fitness coaching membership, to get started you will be required to complete a (1) Fitness Coaching Intake (2) Fitness Coaching Agreement (3) Total Transformation Coaching (optional, only for those who selected this program for added nutrition and lifestyle coaching). For parents registering a minor, you must have had minor's attendance during consultation. Parent's/legal guardians are required to complete a (1) Parental Consent & Waiver and (2) Fitness Coaching Agreement on behalf of the minor. Follow the instructions below to complete all necessary documentation forms.

All forms must be completed and submitted in advance prior to the start of any services being carried out for the safety and transparency of all individual's and their best interest in their wellness journey as we strive to provide quality services.

Incomplete or missing form submission may result in cancellation of your session without a refund, no exceptions. This is to protect you and your health history and the prevention of injury while under our therapeutic care or fitness coaching.

MHH Massage New Client Intake
Select the "MHH Massage New Client Intake" form below if you're preparing to receive a Therapeutic Massage with MHH Massage.
(all individual's must complete this form prior to receiving any massage services , and failure to do so may result in an instant cancellation of your session with NO refund.
Prenatal & Alternative Therapy Forms
Select the "Prenatal Intake & Consent" form below if you're preparing to receive a Prenatal Massage with MHH Massage.
(all individual women must ensure you are at minimum or beyond your 1st trimester (12 weeks or more) before consenting - no exceptions)
Select the "Reiki Intake & Consent" form below if you're preparing to receive Reiki Balance Therapy with MHH Massage.
(all individuals must have some knowledge of what reiki is and what to except concerning side effects and treatment reasons)
Fitness Coaching Forms
Select the "Fitness Coaching Intake" form below for those preparing for Fitness Coaching with Starr Fit Club.
(all individuals must also complete the "coaching agreement" form before the start of your first initial coaching session)
